60yr old female presented with abdominal pain from 2-3 months to general medicine department

A 60yr old female presented with abdominal pain, vomitings and loose stools.

CHIEF COMPLAINT: Abdominal pain from                                      2-3 months,
                               Vomitings 11 days back, 
                            loose stools 11 days back.

HOPI: The patient presented with abdominal pain which was localised to epigastrium and left hypochondrium.The pain is inscidious in onset ,gradually progressive and intermittent and of  burning type.
- The pain aggravated by sitting or empty stomach and relieved by taking some unknown medication.
- 11 days back she had 3 episodes of vomitings which mainly had food as content , non- bilious, with no blood and is non projectile.
-11 days back she even had 2 episodes of loose stools which were not foul smelling.
- Associated with low urine output for the past one day, generalized body weakness, cough with sputum, no outside food intake, no fever, no cold , no chestpain , no headache ,no burning micturation.

PAST HISTORY: She had a similar type of abdominal pain 2yrs back and went to a local hospital in chityal and was treated with some unknown medication and the pain got relieved.11 days back she had a sudden onset of similar episode and because of unbearable abdominal pain she consumed a herbicidal poison because of which she was having repeated episodes of vomitings and loose stools.She is not a known case of HTN, DM,Tb and asthma.

PERSONAL HISTORY: Diet - mixed 
                                     Sleep- N
                                    Appetite-N
                                    Bowel - N

FAMILY HISTORY: No family history

GENERAL EXAMINATION: Poorly built and                                              poorly nourished.

DIAGNOSIS: Multiorgan failure.
